Curious case of 13 year old child.

A 13 year old male patient presented to Kamineni intitute of medical sciences with complaints of leg pain and shortness of breath since 4 days The child was apparently asymptomatic 1 year back. Then his guardians noticed that he was gaining weight . The child mentioned that he gained around 15 kgs in that one year. (from 35 kgs to 50 kgs). Along with that, he developed shortness of breath . It was grade 1 breathlessness ( mMRC classification ) which did not progress. 6 days back , the child developed moderate, continuous and dragging type of pain in the legs extending down from the knees bilaterally. The child's sleep was disturbed due to pain. It relieved only on medications. There is H/o fever 2 days back which was low grade intermittent fever that subsided on medication. There is no H/o cough, wheeze, chest pain, palpitations, syncopal attacks, dizziness There is no H/o vomiting, headache, anorexia, burning micturition, abdominal pain or decreased urine output. ...

Case of Heart failure with cirrhosis of liver

A 69 year old male patient presented with Shortness of breath and dry cough since 4 days. 2 years back he began developing pedal edema which began in the left foot and later involved right foot also. Gradually the edema extended and the entire lower limbs were involved. Dyspnea progressed from grade 2 to grade 4 (MMRC classification). There is history of orthopnea. There's history of anorexia. No H/o chest pain, palpitations, syncopal attacks, dizziness. No H/o leg pains, burning micturition, oliguria, fever No H/o altered sensorium, constipation nor diarrhoea Pt is a k/c/o type-2 DM since 26 years. He is on oral hypoglycaemic agents since then. Pt is a k/c/o hypertension since 15 years. He is on oral antihypertensives. Patient developed joint pains 25 years back that led to joint deformities of interphalangeal joints. The pains subsided spontaneously. Pt is not an alcoholic nor a smoker. On examination : There's no pallor, icterus, cyanosis, clu...
